New Patients

We're glad you're here. Starting care can feel like a big step — we want to make it as smooth and welcoming as possible. Here's everything you need to know before your first visit.

Your First Appointment, Step by Step

Request an AppointmentCall our office or use the scheduling link to request your initial evaluation. A team member will confirm your appointment and send intake paperwork.

Complete Intake FormsYou'll receive intake forms by email before your visit. Please complete them at least 24 hours in advance so we can review your history and make the most of your time together.

Verify Your InsuranceIf you plan to use insurance, contact your provider to confirm your mental health benefits. See our Insurance & Fees page for a list of accepted plans and self-pay rates.

Attend Your EvaluationYour initial evaluation is 60–75 minutes. Dominique will take time to understand your history, current concerns, and goals — this is a collaborative conversation, not a checklist.

What to Bring

Photo ID (driver's license or state ID)
Insurance card (if using insurance)
List of current medications, dosages, and prescribing providers
Names and contact info for any current or previous providers
Relevant medical or psychiatric records, if available
Payment method for copay or self-pay balance

A Note About Medications

If you are currently taking psychiatric medications, please do not stop them before your appointment. Bring a complete list of what you take, including dosages and how long you've been taking them. If you have questions about your current medications, write them down — your first visit is a great time to ask.

In-Person & Telehealth

We offer both in-person appointments at our Clinton, MS, office and telehealth visits via secure video for patients located in Mississippi. Telehealth is a convenient option for follow-up appointments and medication management.
